NHS Patient Survey
At Simply Pharmacy we always try to provide you with the best possible service.
Each year we ask a random selection of NHS patients to fill in a short questionnaire about the service we provide. The questionnaire is anonymous.
Please help us by completing the online questionnaire. Your views are important to us. It will only take a few minutes to complete.
You may contact the pharmacy by telephone and we will provide you with the link to the online survey. Alternatively we can post hard copies out to your home address.
Once the survey is complete a copy of the Survey Report will be available on the website.
We will use the results of this survey to improve the services we provide at this pharmacy.

As a result of the Covid-19 Pandemic, the annual Community Pharmacy Patient Questionairre was suspended.
Survey Results Summary 2022-2023
As from October 2022, the annual patient satisfaction survey is no longer required to be completed.
